    Alternatively, claims can be reported via email at pmamcfr@pmagroup.com or faxed to 888-329-2721. A representative at the customer service center can also take claims over the phone at 888-476-2669.

  • Worker’s Compensation: Worker’s Comp claims can be reported to A.I.M Mutual online at https://www.aimmutual.com/policyholders/report-an-injury/massachusetts.html, via phone at 866-270-3357, or by fax to 781-270-5599.

Clergy-Most vehicle registration renewals can now be processed online. If your vehicle registration requires an insurance stamp for the Massachusetts Registry of Motor Vehicles to process your renewal application, please mail your vehicle registration renewal form to:

Before mailing-make sure to sign the back of your vehicle registration renewal form, enter your license number and include a check made payable to:  MassDOT for the amount of your registration renewal. If you do not provide all of the aforementioned items, your registration renewal cannot be processed. Please send a copy of all vehicle registrations, each time they are renewed, to a.patry@diospringfield.org. Failure to do so could result in a lapse in coverage and/or registration. Please note that the Finance office does not have the insurance stamp and any documents requiring the stamp must go through Mary Ann.
Certificates of Insurance: To request a Certificate of Insurance for General Liability or Property, you must fill out the form below and email it to michael@wrs1928.com. It is helpful if you can also send the expired/expiring Certificate of Insurance for reference.
Requests for a Workers Compensation Certificate of Insurance should be emailed to Amanda Taillon at Amanda_Taillon@rcab.org.
Special Event Insurance – If Parishes/Schools rent out their property, whether for meetings or events, renters must have Special Event Insurance. This includes receptions held after funerals and all non-parish sponsored group meetings. Special Event Insurance is a protection for the Parish and the Diocese in case of accidents. Please email a completed Special Event Insurance Request Form and a Facilities Usage Indemnity Agreement to a.patry@diospringfield.org to purchase Special Event Insurance.
*Renters, if you’re interested in renting a parish hall, please contact the office of the parish directly.
